Bury Community Service is looking for motivated individuals to provide a high standard of care, utilising specialist skills in the assessment, planning and implementing pro-active care needs to a designated neighbourhood population with complex needs and/or Long Term Conditions.

Working in collaboration with Primary and Secondary care clinicians members of the multidisciplinary and Integrated Neighbourhood Team, liaise with the wider teams hospital, other professionals and voluntary agencies to ensure care is patient centred and appropriate to their assessed needs. The post holder will be supported clinically by the District Nurse Lead and is accountable to the Integrated Neighbourhood District Nurse lead the caseload is shared by all members of the Integrated Neighbourhood Team.

The post holder will be able to demonstrate their professional skills and competence in leadership, teamwork, delegation, care planning / co-ordination of individual’s needs medicine management, and health and case management.

Communications and Relationships
1. Promote good communication links in line with the Trust communication policy.
2. Ensure effective communication of complex, highly sensitive and emotional information to at risk/vulnerable patients requiring excellent interpersonal, negotiation and counselling skills.
3. Ensure seamless delivery of care by demonstrating excellent interpersonal skills (written and verbal) in order to liaise with primary, secondary and independent and third sector professionals.
4. Empower patients and their families through appropriate support and education. Facilitate the integration of patient goals for health and social care into current care plans and future health needs.
5. Set appropriate aims and objectives in negotiation with the patients to give comprehensive information and advice to seek patients understanding, co-operation, compliance and consent.
6. Provide planned and spontaneous advice to patients / clients, junior staff, staff of other disciplines, formal and informal carers and relatives.
7. Have the listening and observational skills to concentrate on the individual needs of patients and plan, document and evaluate on the information gathered.
8. Deal with distressing or emotional situations, such as dealing with patients and families who are distressed due to long term conditions or end of life.
9. Maintain accurate contemporaneous treatment records of patients' care and provide data for statistical purpose.
10. Maintain and update patient’s records and record clinical activity electronically in accordance with trust policy.

Analytical and Judgemental Skills
1. To undertake data collection effectively using the agreed systems
2. Have the ability to determine, based on the information provided within the triage system, on how to prioritise and schedule interventions when working with Single Point of Access (ACNT)
3. To use clinical judgemental skills to decide upon and recommend best course of action for assessing and interpreting complex needs of patients/families/carers when a range of options exist, escalating as appropriate
4. Analyse situations and instigate emergency procedures as required

Planning and Organisational Skills
1. Participate in professional and policy making activities that contribute to the development of the community matron role and services.
2. Participate in evaluating all aspects of clinical care, identifying areas for improvement and contribute to bringing about change
