PRIMARY DUTIES & AREAS OF RESPONSIBILITY To be professionally responsible and accountable for the management of own clinical caseload and to organise this effectively with regards to clinical priority and effective use of time. To work as a team with other oncology dietitians to provide dietetic care to oncology and haematology inpatients and outpatients referred by Oncology Services. To manage a specialist caseload of patients referred to the Enhanced Supportive Care service within the Trust and contributes to the multidisciplinary team. To provide regular out-patient dietetic service to patients undergoing treatment including radiotherapy, chemotherapy and attending general consultant led oncology / haematology clinics.

To provide support on request to patients attending the Mustard Tree Cancer Support Centre, including palliative care and supporting the weekly dietetic clinic. To attend ward rounds and clinical team meetings as appropriate to ensure that expertise contributes to multi-professional decisions on care. To work autonomously to provide highly complex advice to patients, carers and healthcare professionals. This will involve nutritional assessment (based on interpretation of biochemistry, anthropometry, clinical condition and dietary history) and development of nutritional care plans which are both realistic, practical and evidence based (based on assessment of medical, social, economic, cultural and psychological factors).

To monitor and review the outcomes of nutritional care plans and adjust to facilitate achievement of the desired goals in the light of changing medical treatment and personal circumstances To communicate effectively with patients and carers to maximise condition management, recovery potential, further disease prevention and during terminal care To formulate feeding regimes and prescribe a wide range of artificial feeds based on medical condition, fluid management, biochemical and anthropometric monitoring. To demonstrate a good working knowledge of types of enteral feeding tubes and the required care for tube patency. To supervise and co-ordinate the safe and timely discharge of patients on artificial nutritional support, liaising with hospital and community services To work with other dietitians within the team and cover for other staff as requested by the Team Leader or Professional Lead. To advise on appropriate use of prescribable nutritional products within the oncology and haematology service and to general practitioners.

To liaise with housekeeping and catering services over the provision of suitable meals for oncology patients. To maintain accurate and timely dietetic patient records in line with national and local record keeping standards. To participate in the production and review of resources to ensure that they are evidence based, up-to-date and in line with Trust Document Control Policy. Organise, deliver and develop education and training programmes for patients, families/ carers, nurses, doctors, students and other MDT team members.

Deliver formal and informal education sessions to groups as requested. Participate in the training and supervision of student dietitians To train, supervise and appraise junior members within the team. To keep caseload/activity statistics as required by the Department of Nutrition and Dietetics Professional To be professionally and legally responsible and accountable for all aspects of your own work, including the management of patients in your care. To determine your own work priorities, balancing clinical, professional and organisational demands.

To take a flexible approach to supporting colleagues in times of caseload pressure, including working with colleagues to maintain clinical cover across the team within the resources available. Adapt to changing demands of the job to provide a dietetic service in the absence of co-workers. In collaboration with other members of the Nutrition and Dietetic team, develop and implement department practice changes, policies, protocols and guidelines as required. To attend, and contribute to Dietetic Department Meetings To participate in departmental appraisal on an annual basis.

Any other duties appropriate to the grade as requested by line manager or Professional Lead. COMMUNICATIONS & WORKING RELATIONSHIPS It is essential that the post holder is able to communicate with a wide range of personnel Department colleagues, student dietitians, local/regional dietitians and occasionally other dietitians nationally Medical, nursing, paramedical, catering and administration staff Patients, relatives and carers - Receives information concerning patients history, sensitive issues, some patients with little English; requires persuasive, reassurance skills Gaining consent and understanding from patients or their family/carers of the treatment plans required.
