We are expanding the existing workforce by recruiting to the current service portfolio and to the new teams.

The post holder for this role will be integral to the delivery of an effective and competent therapy programme set up by a multidisciplinary therapy team and will include supporting the work of OTs, PTs, SALTS and other team members.
- To carry out enablement programmes as delegated by the MDT (Multidisciplinary Team - Occupational Therapists, Physiotherapists, Nursing staff, Pharmacists, Dietetics and Speech and Language Therapists). This will be directed by the patient’s rehabilitation plan.
- To promptly report any enablement programme concerns to an appropriate professional.
- To consistently deliver a patient-focused enablement service that promotes optimum independence.
- To plan, participate in and run therapeutic groups with guidance.
- To adhere to the Trust code of conduct and policies.
- To attend mandatory training and contribute to in-service training.
- To participate with data collection and service development

Central London Community Healthcare (CLCH) is one of the largest community healthcare organisations in London and Hertfordshire, providing our services to diverse communities/boroughs in 11 London Boroughs - Barnet, Brent, Ealing, Hammersmith & Fulham, Harrow, Hounslow, Kensington and Chelsea, Merton, Richmond, Wandsworth, Westminster - and Hertfordshire.
- To undertake enablement programmes in which you have previously demonstrated competency within the inpatient or community setting
- Monitor the patient during treatment sessions and feedback outcomes of enablement sessions to the relevant MDT members. Report any untoward incident to a qualified member of the MDT immediately.
- To maintain patient safety at all times and support continued improvements in care such as working to reduce falls and pressure ulcers
- To adhere to universal precautions and infection control guidelines, including hand washing and disposal of waste, and ensure effective cleaning of equipment before and after the session
- To be responsible for ordering appropriate equipment for patients as instructed by the MDT
- To handle and move patients, as indicated, by using prescribed equipment and techniques as detailed in training and be familiar with, and utilise, the services and advise given by the Handling Advisor
- To contribute to the protection of vulnerable adults and children by highlighting and monitoring for, signs of abuse and reporting these to qualified member of the MDT
- To constructively respond to patients questions and promptly pass on any you are unable to resolve to the appropriate professionals.
- To participate in enabling patients to maintain, and be introduced to new leisure, interests and relationships.
- To effectively manage your own time for designated duties flexibly, being able to reorganise your day as instructed to respond to the changing needs of the patients and service.
- To plan, participate in and run therapeutic groups with guidance.
- To carry out drugs adherence assessment for new patients as directed by the pharmacist.
