This permanent full time role (37.5 hours) works to deliver service 7 days per week from 8am-10pm (including bank holidays). Part-time will be considered.

The role of the Advanced Clinical Practitioner is to work across professional boundaries, undertaking a variety of clinical duties, within the context of the clinical team and to work as part of the community response team and work with the principles of the Community Services Division. The Advanced Practitioner is required to work as an autonomous clinical professional.
Within the multi-disciplinary team, the Advanced Practitioner will support the provision of high quality patient centred health care. To identify, assess, diagnose and treat adults with acute illness / complex needs and long term conditions. This includes supporting our Community Assessment Unit at the acute site.
Dependent on knowledge skills and experience, you will be intrinsically part of the Community React Team providing expert clinical advice to inform sound clinical judgement in the diagnosis and continuity of assessment, decision making, treatment and management of patients in the community. You will help to lead the clinical development of the service in partnership with the Consultant team, providing knowledge of clinical governance and educational development of others.
We are looking to recruit an ACP to work within the Community React Team (CRT). The post holders will work as part of the Community multi-disciplinary teams to manage patients in the community setting, including patients' home, care homes and the SDEC ambulatory chairs at the acute hospital site. The post holders will support the clinical team in terms of clinical leadership and expertise.
The CRT works in collaboration with North West Ambulance Service (NWAS), Primary Care, Care Homes and the wider healthcare economy, with a focus on safe admission avoidance.
This is an exciting time for the Community React Team as we continue to evolve, working closely with our key stakeholders. The expectation is to work within a changing environment and to drive the transformation agenda.
You must be able to travel across Trust independently.
An Enhanced DBS with Adults' Barred List checks will be obtained during pre-employment checks.
This post involves contact with patients and families face to face within a community setting. Therefore it cannot be ruled out that the postholder may come in to contact with people who have COVID-19, although the Trust provides PPE and takes all reasonable steps to minimise risk.

**The post holder will**:
- Work as an advanced practitioner making independent clinical judgement/decisions based on patients’ clinical needs.
- Act as a lead for the development of advanced and specialist practice.
- Undertake independent comprehensive assessment and management of patients to incorporate physiological and pharmacological considerations, making multifaceted complex clinical decisions regarding patient management and clinical outcomes.
- Demonstrate a high level of competence in clinical, diagnostic and examination skills.
- Deliver a proactive model of service delivery for the management of patients and carers within the community and care home setting with complex care needs.
- Provide counselling and advocacy for patients with acute and chronic conditions.
- Undertake research and service development.
- Establish close liaison and communication between all stakeholders across health and social care, community, primary care and third sector disciplines involved in the care of patients and their families and work in partnership with other practitioners.
