Are you a motivated, dynamic, and enthusiastic registered Nurse or Therapist with significant experience of working with people that are homeless or rough sleeping?

If so, we have an exciting opportunity for you to be involved in the process of re-establishing our specialist Homeless Health team. We have been awarded three years funding to continue the development of the service.

The Homeless Health Service provides enhanced inpatient care, and ensures safe, and sustainable hospital discharges that meets the needs of patient’s currently experiencing homelessness.

You will provide operational management support to the Homeless Health team and will work closely with the GP Clinical Lead in the evaluation and ongoing development of the service.

A strong understanding of how self-neglect is part of homelessness, safeguarding and the complexities of undertaking mental capacity assessments in inclusion health groups is desirable. A positive, can do ‘attitude’, a commitment to changing care for this extremely vulnerable patient group, and a desire to communicate with a wide variety of partners and stakeholders is essential.

You will receive specialist support and supervision from the Imperial integrated care directorate management team and the charity Pathway - healthcare for homeless people, with whom we have a strategic partnership.

The Homeless Health Team works in close partnership with the wards to identify people experiencing homelessness whilst they are in hospital, build therapeutic relationships with them, assist the ward multidisciplinary team to maximise the benefit of these admissions, and then support these vulnerable patients during and after discharge - ensuring that they are fully linked in to all the health care and support services they need when the team finally discharges them.

The Homeless Health team provides a service to all sites across ICHNT. The team also provides remote specialist advice to Chelsea & Westminster hospital.
