Ribble Valley Community Mental Health Team are seeking to appoint a Band 5 Community Mental Health Practitioner with the aim to progress within 12 months to a band 6 post. This post provides the opportunity for a forward thinking and experienced individual to develop and implement safe, sound and supportive services that embrace the provision of holistic care. With a strong emphasis on multidisciplinary working and the principles laid out in the Recovery Model, you will need to be an innovative thinker and be able to form effective relationships.

You must be a registered mental health practitioner, with excellent leadership and clinical skills. You will also need to have a working knowledge of the care programme approach, risk assessment and management, and the ability to explore issues relating to the impact of health and social needs on mental health and well-being
1. To undertake mental health assessments including risk assessments within a multi-disciplinary assessment team. Working closely with the Consultant Psychiatrist, thus ensuring that there is a multidisciplinary team approach
2. To be proficient in referring to other clinicians within the service or referring to other services/agencies where necessary.
3. To provide a fast, responsive service to assess individuals needs for access into mental health services
4. Providing a communication structure between GPs, IAPT and secondary MH services in LCFT, non-statutory service to ensure the highest possible standards of patient care
5. To provide mentoring and support for others

Lancashire & South Cumbria NHS Foundation Trust provides a range of services including secondary mental health care across this area. Specialist provision comprises inpatient child and adolescent mental health services, perinatal mental health and forensic services including low and medium secure care.

The Trust also provides a range of physical health and well-being services in the community alongside a range of partners in the Lancashire, Cumbria and Sefton area.
