This is an exciting opportunity for a mental health nurse or social worker to join our Trafford based early intervention for psychosis service.
The role will focus of the effective management of a caseload of people with complex mental health and social care needs, using evidence-based practice to assess, plan, implement and evaluate interventions and document these in accordance with local and national standards and guidance.
To have care-coordination responsibilities for an agreed number of clients and to participate in the assessment of people referred to the team with possible first episode psychosis.
Early intervention services aim to reduce the stigma associated with psychosis.
Improve professional and lay awareness of the symptoms of psychosis and the need for early assessment. Reduce the length of time young people remain undiagnosed and untreated. Develop meaningful engagement, provide evidence-based interventions and promote recovery during the early stage of Psychosis. Increase stability in the lives of service users, facilitate development and personal fulfilment.
Provide a user led service, i.e. a seamless service available for those from age 14 to 35 years (on acceptance) that effectively integrates child, adolescent and adult mental health services and works in partnership with primary care, education, social services, youth and other services. To ensure that care is transferred thoughtfully and effectively at the end of involvement.
We are a very dedicated, highly motivated and dynamic team. We offer a range of psychological therapies/psychosocial Interventions and have a comprehensive training programme across the directorate.
We have regular in house training and clinical supervision with our two Clinical Psychologists and an experienced and supportive leadership team. We work very closely with Service users and families, offering family interventions and carer education/support. We value Service User involvement and encourage participation in service developments on many levels including opportunities for peer support roles.
This is an exciting opportunity for a Care coordinator to join our specialist service for People experiencing the First Episode of Psychosis.
