The post-holder will undertake advanced clinical assessment, needs identification, planning, case management and co-ordination of care for individuals with highly complex needs and co-existing long-term conditions. The post-holder will be able to demonstrate advanced professional practice including advanced clinical skills, medicines management, leadership, supporting self-care management and forge interagency and partnership working to improve outcomes for patients with co-existing long-term conditions. In addition, the post-holder will work proactively to reduce unplanned hospital admissions and facilitate earlier discharge from hospital for patients identified through multi-disciplinary team meetings, high risk patient programmes or proactive care management. To role model compassionate and inclusive leadership in order to shape the creation of a collective leadership culture within the trust.

This means demonstrating a consistent leadership style which (a) engages, enables and empowers others (b) uses coaching to promote ownership of learning and quality improvement and (c) facilitates team working and collaboration within teams / departments and across organisational boundaries.
