Assistant Practitioner

Due to recent relocation of our band 4 we are now looking to recruit an enthusiastic, motivated Band 4 Assistant Practitioner or Nursing Associate to work within the Integrated Health Team in Stratford. We are really fortunate to be based on the Stratford hospital site. Our work area includes Stratford upon and the surrounding rural villages.
As an Assistant practitioner /Nurse Associate you will need to be flexible and hardworking as part of your role you will be seeing patients in their own home environment, under mínimal supervision once you have demonstrated your clinical competencies adhering to SWUFT's policies.
Working within a variety of staff groups communication skills are a key skill.
There is a requirement to have good IT skills as the Trust uses electronic patients records.
You will be required to have completed or about to complete the Foundation Degree in Health and Social Care or equivalent higher apprenticeship. The nursing associate has a breadth of knowledge across the lifespan and across the fields of nursing, providing holistic and person centred care and support for people of all ages and in a variety of settings. The nursing associate works independently under the leadership of registered nurses,working within the sphere of nursing and care and within all aspects of the nursing process.
Nursing associates are equipped with the knowledge, understanding, skills, attitudes and behaviours relevant to employment as a nursing associate and will work to a nationally recognised code of conduct.
