Primary Care Mental Health Practitioner B6: North Kerrier West Primary Care Network

It’s a refreshing & exciting time to join our Adult Mental Health Services in Cornwall, as we are focused on enhancing our services with outstanding levels of quality care. Our wholescale transformation of the core community mental health service offer is well under way, and we are looking for creative people to join our workforce. We are forging and renewing partnerships between colleagues in Primary & Secondary Care Services to create innovative and transformed mental health care for our population.

Due to these positive & transformative changes, we now have a stimulating and unique opportunity for the right person to work along with the existing mental health team within North Kerrier West Primary Care Network. Whilst this will be under the employment of Cornwall Foundation Trust (CFT), the post is practice based and supervised by the PCN’s Mental Health Practitioners who are also practice based.

The PCN patient population is circa 40k spreading across a large area of mid-west Cornwall from our glorious coastline to our heritage moorland and mines. There are two member practices: Carn to Coast Health Centres and Praze Surgery (now known as Godolphin Health).

**Main duties of the job**:
Our PCN is currently one of only a few in SW England to embrace a novel Artificial Intelligence powered triage system KLINIK which is designed to support clinician triage, patient navigation and improve both the patient and staff experience. Our mental health team support the practices’ duty teams in managing on-the-day demand and there may be opportunities for triaging patients utilising the Klinik system, as well as supporting those patients who can be effectively and appropriately managed within primary care with clinically required follow-up.

The post holder will be well supported within the PCN, within the emerging West Independent Care Area (ICA), which has a strong collaborative ethos, and across Cornwall as appropriate. Our PCN already employs three mental health practitioners so you would be joining an established team.

We work very well as a PCN and both have excellent supportive teams of clinicians and administration staff. Both practices are GP training practices, and we fervently support personal professional development. Salary and hours to be a
