SuttonInAshfield, United Kingdom Nottinghamshire Healthcare NHS Foundation Trust Full time

An exciting opportunity has arisen for dedicated and enthusiastic mental health practitioners to join our growing team of multidisciplinary clinicians providing care to patients within the Primary Care Network (PCN) footprints of Nottingham and Nottinghamshire. These are pivotal roles working across primary and specialist services within an integrated community mental health model.

In Nottingham and Nottinghamshire there are 23 PCNs with a variety of different sizes, patient populations and mental health priorities. During 2021, each PCN will have at least one mental health practitioner providing a named clinician for the GP practices within that locality.

The posts will be co-located within the PCN and be responsible for providing specialist mental health care and interventions to patients within the PCN using high level assessment to understand needs, treatment options, refer appropriately and co-ordinate the patient’s journey across the health, social care and voluntary sector system, regardless of care setting. Its essential that these posts forge close working relationships with a range of professionals both within the PCN and local Mental Health Trust to enable comprehensive and patient centred care plans to be developed and delivered.

We have a huge variety of opportunities in our expansive Trust covering geographically from South Yorkshire, Nottinghamshire to Leicester, providing mental health inpatient and community services, community health services, forensic mental health services and secure prison healthcare across the East Midlands.

We have more than 100 sites and a huge scope of unique opportunities to develop your career and gain some amazing experiences. We strive to be a great place to work and offer many staff benefits and tailored staff support and wellbeing programmes.

Do you want to make a difference?

Then Nottinghamshire Healthcare is the perfect place for you to start, maintain or further your career. Join our team of nearly 9000 who are making a difference every day. We are all about our people - our staff, volunteers, carers, service users and patients. We are NottsHC.

The roles will be hosted by Local/Community Mental Health Teams but work across Primary Care Networks and we have vacancies across Nottinghamshire covering the following areas:

- Newark & Sherwood
- Mansfield & Ashfield
- Bassetlaw

This is a unique opportunity to work across both primary care and specialist mental health services where you will be supported to develop new ways of working to support service users and their families

**The post holder will**:
Provide advice, enhanced assessment, support and treatment for those experiencing problems with their mental health and emotional wellbeing as part of a multidisciplinary team.

Provide support to theall age adult mental health model.

Provide clinical advice and support to non-mental health clinicians in a PCN.

Be part of the interface between primary care and specialist mental health services in the Locality Team.

Relevant professional qualification (RMN, RNLD, OT, SW)

Significant post qualification experience within the management of patients with complex needs

Excellent communication and liaison skills with the ability to work autonomously

Primary care experience is not essential but an understanding of how PCNs work would be beneficial

In addition the post holder will need to have excellent managerial and leadership skills and will be expected to support service transformation by leading on service change to embed new ways of working across primary and secondary care.
