As a Social Care Assessor, you'll be responsible for assessing and reviewing clients in our home care service, from initial assessments when they join the service, to more thorough evaluations for more complicated cases.

You will provide excellent client service, working collaboratively with the individual, liaising with their family and other professionals to develop a care plan that meets the individual's needs, as well as identifying risks, and how these can be managed safely.

**Location**: Lancaster/Morecambe
- Flexible working hours, in line with service requirements
- Opportunity exists for part-time working
- Company phone and laptop

Specialist Care Team is a social care provider that operates across North Lancashire in the following areas: homecare, supported living and residential. Our client groups cover all adults, including older people needs, end of life, physical disability, learning disability and autism and mental health.

In your role, you will be responsive and meet with the clients, their relatives and other professionals to fully assess the support they require and develop a person-centred, comprehensive care plan and risk assessment.

Over the first few weeks, you will carry out your assessment and complete the care plan, which may include carrying out some visit tasks so you can really understand the individual's needs. You will liaise with the homecare team during this period; ultimately handing the client over to the team.

Responsibilities include but are not limited to;
- Welcoming new clients to the service, explaining the role and limitations of the team and agreeing objectives
- New client assessments
- Writing care plans
- Maintaining care plans and carrying out reviews
- Liasing with the homecare management team to ensure staff/client compatibility

**We are looking for someone who**:

- Has experience of undertaking care assessments
- Ideally qualified to Level 3 or suitably qualified through care experience
- Has excellent communication skills
- Holds a driving licence and has access to a vehicle
- Is personable and trustworthy, to instil confidence in the service
