An exciting opportunity has arisen to join the Guys and St Thomas' Community Stroke Team as a Band 5 Stroke Care Advisor. Our Community Stroke Team is part of the Guys and St Thomas' Community Neuro-Rehabilitation Service and consists of a Deputy Clinical Lead, Clinical Psychology, Physiotherapists, Occupational Therapists, Speech and Language Therapists, Stroke Care Advisor, Community Stroke Nurse, Stroke 6-month reviewer, Occupational Therapy Apprentice and Rehabilitation Support Workers.

The Stroke Care Advisor works with the community stroke and neuro-rehabilitation teams to help Lambeth residents who have had a stroke; promoting well-being and independence and supporting stroke survivors, their families and carers to access a variety of services in the community. We follow a self-management model, encouraging patients to participate actively in their rehabilitation and to continue on their own after our input has finished.

This post would be suitable for someone who has experience of working with stroke survivors and/or adults with physical and cognitive impairments. Effective communication and interpersonal skills are essential to the role as well as having experience of working as part of a team and the ability to work independently. You will require good organisational and IT skills in order to effectively manage your own time and workload. We have community bases around Lambeth and also utilise agile working.

Please note you do not need to be able to drive for this position. If you do drive, there are pool cars available or you can use your own car. You would otherwise use public transport, own bicycle, or walk. Travel expenses for work-related travel are reimbursed.

Job Summary
- To work as an autonomous stroke advisor with a holistic approach in supporting patients with stroke recovery.
- Supporting people who have experienced a stroke, and their families, to adjust to their new situation.
- To carry out assessments of individual care needs for people with stroke and their carers.
- Support and establish peer group and social activities for the local stroke community
- To be responsible for the collation of data of service users known to the Neuro Rehabilitation service in Lambeth that are referred for stroke advisor input
- To maintain an effective referral process and administrative practice working collaboratively with the multidisciplinary stroke rehabilitation team.
- To record data relevant to the Lambeth community stroke service and be actively involved in analysing the reports for local and national services.
- To work autonomously and in conjunction with team colleagues across a variety of locations within the community setting, including in clients' homes, in order to meet the individual needs of adults living with stroke with physical and/or communication disabilities.
- To work across borough, accessing and recording information using mobile devices, and coming into offices for meetings or specific events.
- The post holder must use Skyguard and other personal protective equipment as determined by the service to ensure safety in general and to protect lone workers in particular.
- To support clients' carers in finding suitable ways of conducting their role and identifying areas in which they might need support.
- To represent the department in internal and external meetings as appropriate and to take part in training and mentoring/supervision of
