Discharge Hub Nurse

The Discharge Hub is looking for an experienced Band 4 Assistant Practitioner to work within a triaging multi-disciplinary team. The Discharge Hub is an exciting joint venture for Torbay and South Devon NHS Foundation Trust and Devon County Council. The Hub receives referrals from the Acute and Community Hospitals in Torbay and South Devon, plus West Devon, Derriford and The Royal Devon and Exeter Hospital.

The work is varied and extremely patient
- focused to ensure that the person receives the onward support that matches their needs and profile. We are looking for the post holder to work with the team to triage the complex nursing cases and continually work to improve the quality of the service provided. The post holder will be required to undertake delegated assessments, with support from the occupational therapists in the community or hospital settings e.g. support localities for ‘Discharge to Assess’. The post holder will need sound clinical reasoning skills, enjoy a challenge and have a good knowledge of Acute and Community Services. The Hub team is diverse and a very supportive unit. The post holder would work very closely with their acute and community colleagues who include the Complex Hospital Discharge Team, Therapists, Nurses, Social Workers and Discharge Co-ordinators. Ongoing training will be provided and development opportunities are available.

To keep the patient at the centre of his or her care, ensuring patient and carer participation in decision making.

To practice the principles and philosophy of individualised person-centred patient care in accordance with Trust Policy, procedures and regulatory body’s guidelines.

To maintain accurate and contemporaneous clinical records.

To record activity on a clinical data base as required.

To work within a multi-professional team undertaking practitioner role, supporting work of all staff including Nurses, Occupational Therapists, Health and Social Care Coordinators and Social Care Assessors.

To work with the wider locality teams and onward services, working together to ensure patients’ needs are met by the right person at the right time.

Communicate with both colleagues and patients with humanity and compassionate as you would expect from others, showing empathy and kindness to everyone.

Maintain a sensitive approach in emotionally challenging situations, e.g. palliative care.

To undertake the triage of referrals presented to the Discharge Hub and work as part of an MDT approach to advice of the most appropriate short-term offer to support the patient’s identified needs.

To use your experience and clinical skills to aid the MDT in the Discharge Hub to problem solve and match onward services to patient profiles.

Be able to formulate discharge plans to meet the patient’s needs with information from the Acute and Community Hospitals and match with the availability of the services within the community.

To undertake delegated assessments, with support from the occupational therapists in the community or hospital settings e.g. support localities for ‘Discharge to Assess’ patients, in order to contribute to excellent patient care and support hospital discharges.

To assess for and supply necessary equipment from our community equipment providers, to support hospital discharge and the discharge to assess model.

Wherever possible to contribute to expediting hospital discharges and reduce inpatient length of stay to ensure that patients receive the appropriate care in the most appropriate setting.

Act as an autonomous practitioner evidencing problem solving, decision making and independent judgement in a variety of community and office-based settings and situations.
