Community Team Leader Nurse

Daytime Healthcare are currently working with a Hospice who are currently looking for a Community Team Lead to join their team

You will be expected to work in the community covering Hackney, Tower Hamlets and Newham. Senior Hospital Nurses and Team Leaders who wish to transfer into a community setting will be considered. This is a Band 8 position and you will be expected to leada team of 10 in the community.

The Role;
- You will need to provide palliative care for patients with cancer and other life-threatening conditions.
- To provide leadership and vision to support a great team so they can deliver exceptional care.
- You will to act as a leader and support your colleagues' wellbeing and value collabortation.
- To support professional development of staff including guidance and advice.
- The postholder will manage resources efficiently and effectively within agreed budgets.
- The post holder will be responsible for improving the patient experience and safeguarding high standards of care by creating an environment in which excellence in clinical care can flourish and be measured.

Requirements;
- To be a Registered Adult Nurse with an active NMC registration.
- Experience leading a team and a good understanding of collaboration and teamwork.
- You will need to have experience and a very good understanding of pallative care.
- A full UK Driving Licence with access to your own vehicle.
- Attend the multi professional team meetings to provide expert opinion on patient and clinical care issues.
