Leaving Care Key Worker

As a Leaving Care Worker, you will have the opportunity to show your affection and support for young people of ages Between 16 - 25.
You will assist our clients with a range of activities to meet their daily needs and ensure they are supported in their daily life to progress into a positive independent future.
You will also gather information about our clients' likes and dislikes, and provide them with an optimal level of support while working under our company's shared values.
**Responsibilities**:
- Completing daily log books & evaluation sheets.
- Observing and advising the young persons daily habits.
- Conduct duties in compliance with our safeguarding policy, code of conduct, mission statement values, care act and every child matters regulations.
- Communicate with support staff, liase with other organisations to solve problems relating to the young persons welfare.
- Support the young person with education, training, emotional health and wellbeing, mental health and well-being and maintain positivrelationships within the community.
- Comply with municipal, provincial, federal, and nonprofit regulations.
- You will provide support session, with the young person and their support network and aim towards supporting their independence and transition to adulthood.
**Job Type**: Part-time
Part-time hours: 25 per week
**Salary**: £10.00-£11.00 per hour
