An exciting opportunity has arisen to recruit a motivated and enthusiastic Band 3 Health Care Assistant to work with the Community Team, as part of the developing Integrated Care Community (ICC) you will be predominantly based at Brampton & Longtown ICC and part of Carlisle & District ICC, supporting the local population. ICCs aim to address the challenges of an aging population and increasing demand on health and care services by providing more out of hospital care and empowering people to take control of their health and wellbeing. This will require new ways of working to focus on preventing ill health and ensure services are sustainable for the future.

This role requires you to demonstrate the skills and knowledge to support the provision of high quality care, to support patients with complex needs within the community setting.

There is one full time, permanent position available (37.5 hours per week). Daily working hours are between 8:30 and 23:00.

As a Health Care Assistant, working under the direction of a registered health care professional, undertake a range of delegated patient nursing and care duties including clinical observations and recording patient information. Working largely alone and unsupervised.

**Disclosure and Barring Service**:

**Standard Check**: £18 + Administration cost of £3.85 = £21.85. Deducted at £6.85 from 1st month salary and £5 deducted from salary over following 3 months.

**Enhanced Check**: £38 + administration cost of £3.90 = £41.90. Deducted at £11.90 from 1st month salary and £10 deducted from salary over following 3 months.
