Corby, United Kingdom Lakeside Healthcare Full time

**About Lakeside**

**LAKESIDE HEALTHCARE** is changing the face of primary care provision in England. We are bold, adventurous and ambitious and determined to thrive in uncertain times. We are the largest ‘true’ partnership in the NHS and operate from various sites across the East Midlands. We serve the healthcare needs of over 170,000 patients across Northamptonshire, Lincolnshire & Cambridgeshire. Joining our team presents an opportunity to be part of a large organisation that is changing the way primary care is delivered today.

**Living and breathing our values at Lakeside Healthcare**

**Caring & Respect**: Simply put we genuinely care about people: working together for our patients and our teams, our patients come first in everything we do. We strive to ensure we connect and respond to all needs with compassion, care and respect to improve the lives and wellbeing of the communities we serve.

**Teamwork & Quality**: In all areas of our business we network, collaborate and learn from our Patients, Stakeholders and each another to ensure we are always striving to improve, making the right and best decisions to provide the best service.

**About the Practice/Department/Team**

**Rockingham Forest PCN **is a large, forward-thinking Primary Care Network compromising 3 practices across north Northamptonshire. We recognise the value that this role can bring to our practices and our patients, and we look forward to growing our PCN team. Our aim is to provide exemplary patient care; finding innovative solutions in general practice to deliver the best care we can to our patients.

**Job Introduction**

We have an exciting opportunity for an Enhanced Clinical Practitioner to join our team, where you can make a positive contribution to patient care and peer support and supervision.

As an Enhanced Practitioner you will be acting within your professional boundaries, providing care for patients from initial history taking, clinical assessment, diagnosis, treatment, and evaluation of their care with the support of the PCN Clinical Lead as required. You will demonstrate safe, clinical decision-making and expert care for patients within the Primary Care Network (PCN), whilst working autonomously and taking full accountability for patient encounters.

The Enhanced Clinical Practitioner will undertake duties within the Practice itself thus ensuring contemporary clinical skills and credibility as well as ongoing integration with the wider team and services. This would include review and care of patients attending the Practice (for pre-planned services or same day urgent care) and include diagnosing and treating patients presenting with minor illness, seeing, and advising people in respect of their continuing medical and nursing needs.

Your work will demonstrate improvement in patient outcomes, contributing to reduce health inequalities across the PCN and improving the efficiency of general practice and delivering a best-in-class service.

Mentorship and supervision will be provided by the PCN Clinical Lead. The level and type of supervision will be dependent on the post holder’s skills and knowledge and determined by the organisation’s clinical governance arrangements.

Although the expertise of the ECP in Primary Care lies in their ability to operate as a ‘generalist’, notably maternity care is the one domain where legislation stipulates that only a registered medical practitioner or a practicing midwife can provide care for a pregnant woman.

**What you will do**
- An ability to make professional autonomous decisions for which they are accountable.
- Assess and triage patients, including same day triage, and as appropriate provide definitive treatment (including prescribing medications following policy, patient group directives, NICE (national) and local clinical guidelines and local care pathways) or make necessary referrals to other members of the primary care team.
- Screen patients for disease risk factors and early signs of illness.
- Use complex decision making to inform the diagnosis, investigation, complete management of episodes of care within a broad scope of practice.

**Essential Criteria**
- RGM or Paramedic Qualification.
- Current registration with the NMC or HCPC.
- Masters degree - Advanced practice pathway.
- Independent Prescribing Qualification.
- Working at or working towards a master’s level or equivalent that encompasses the four pillars of clinical practice, leadership and management, education, and research, with demonstration of core capabilities and area specific clinical competencies.
- Evidence of Continued professional development portfolio.
- Safeguarding level 3 in Adults & Children & Young People.
- Can work autonomously in an independent and advanced Healthcare role.
- Experience of providing emergency care for patients and management of patients with complex needs.
- Experience in primary care/general practice.
- Evidence of wide knowledge base and competency in clinic
