We are looking for a care co-ordinator to work from our Whitchurch office on a split pattern as a job share with another coordinator.

The role involves 2.5 days in the office, alongside on call responsibilities, worked on a rota basis with the other coordinator.

There are further hours available on the community if desired.

Reporting to the Branch Manager, the Care Co-ordinator is responsible for organising the care services provided by Cross Care. The Care Co-ordinator is responsible for allocating care workers to individuals, producing rotas, liaising with individuals and their families and social workers and ensuring that the correct, high quality services are being delivered.

Purpose of the role:
Organise the care services provided by Cross Care and allocate care workers to individuals, responsible for all data input relating to specified area as changes occur.
- Allocate staff to services, produce staff rotas on a weekly basis and ensure that they are completed and sent out on time.
- Ensure individuals have regular care staff undertaking their care to give continuity.
- Answer telephone calls from individuals and staff and deal with their queries in line with the policies and procedures of Cross Care.
- Update individuals and carer records on an ongoing basis and ensure accuracy of records.
- Contact individuals and staff and communicate necessary information to them regarding the provision of care services.
- Ensure that the policies and procedures of Cross Care are followed by all staff and take appropriate action where necessary in line with the policies and procedures of Cross Care.
- Ensure that any inconsistencies with the policies and procedures of Cross Care are reported to the Branch Manager.
- Taking on new individual referrals from social services, private individuals and other organisations
- Ensure holiday/sickness and emergency calls are assigned
- Allocate care managers to undertake new client assessments.
- Log all compliments and complaints.
- Bring to the attention of the Branch Manager any complaint made about the company or any of its employees.
- Provide on-call cover in conjunction with other care staff. You will be paid an additional rate for on call.
- Attend management meetings as required and contribute to the review of the policies and procedures of the company in order to continually improve the care service provided.
- Where calls cant be allocated, go out and assist staff with the delivery of care.
