We are seeking a motivated and committed individual for the role of the Band 4 Discharge Facilitator on L9A West. L9A West is a busy acute 35 bedded unit for adult patients who have undergone abdominal surgery or have a surgical condition affecting their digestive tract. You will actively participate in assisting with patient flow in and out of the ward by planning the safe discharge of an identified group of patients. This will include all referrals to the community and assessing patients discharge needs on admission. You will work closely with the ward manager, senior ward nurses and the hospital discharge team.

You will be supported to develop a wide range of skills, building upon existing theoretical knowledge, to support the discharge processes required to meet the service needs on L9a west.

Once trained and assessed as competent, you will be expected to work independently, within defined parameters of practice and supervision. You will be able to evaluate the discharge needs of patients liaising with internal multidisciplinary teams and other professionals or agencies. The hours can be flexible but it will predominantly be a Monday to Friday service. The post requires good communication and negotiation skills.

1. The post holder works with support from and access to, but not under the immediate supervision of a registered healthcare practitioner.
2. Participate as an active member of the healthcare team assisting with patient flow and safe discharge of patients as determined by the 9A West senior sister accountable for ‘Patient flow’ and the ward coordinators.
3. To implement and evaluate the discharge needs of patients and to refer for the provision of these care needs to appropriate members of the multidisciplinary team.
4. To supervise and train junior nursing staff, aiding their learning and development of safe and timely discharge planning.

Responsibilities for Patient Care
1. Carry out a range of patient care activities and ancillary duties as delegated by a registered healthcare practitioner and report back on progress/variances. Activities include:

- core clinical support & technical services
- protocol-driven patient assessments
- implementing patient care
- patient support
- administrative
- housekeeping

The role encompasses a number of activities specific to Band 4 APs (see Table A), and some shared with Band 3 HCAs (Table B) - to be undertaken as required.
