An exciting opportunity have arisen to work as a Support Time Recovery worker within the Reconnect -Care After Custody initiative in Cambridgeshire area.

The aim of the service is to ensure that individuals have the equality of access to health by supporting, motivating and empowering them to attend appointments for mental and physical health, including access to substance misuse services.

The role will be based in Cambridge and surrounding areas.

The role may at times include supporting service users in the wider Liaison and Diversion service to ensure a seamless pathway.

The post holder will work as a part of a team that delivers support and services to individuals who are involved in the criminal justice system and also, with offenders as they leave prison and are resettling back into the community.

The post holder is primarily required to work in the community undertaking short term pieces of work with individuals referred to them by senior Liaison and Diversion Service Practitioners or via the Reconnect service.

The post holder is expected to support individuals attend first appointments for assessments by other services such as drug and alcohol services, community mental health teams, autism services, etc.

The aim of the role is to increase access to and uptake up of healthcare and relevant services for people leaving prison or involved in the criminal justice system.
- To reduce health inequalities for prison population and those involved in the criminal justice process.
- To ensure that the health needs of individuals who are leaving prison or Immigration removal centres are met.
- To ensure a safe transition from prison to community healthcare and support services in England and to provide ongoing support so that engagement is maintained.
- To facilitate engagement and liaise with community healthcare and / or other services and enable delivery of a coordinated response.
- To provide advocacy, practical support and to act as a voice for individuals who struggle to engage
- To develop detailed knowledge of community healthcare and other services and to develop relationships so that those involved in the criminal justice process and users of the Reconnect service can access comprehensive and co-ordinated care.
