**Rough Sleeper Outreach Worker**

The role does require assertive outreach work and occasional working outside of "core hours".
- The role will ensure that we are able to provide specialist, tailored support to enable service users to overcome barriers and make positive changes. The role will require you to utilise resources to be able to reach and engage those most marginalised within the community.
- You will have an empathic approach to working with individuals and a great understanding of the needs and barriers of the client group.
- You will take a client centred approach, ensuring that every client’s package of care encompasses all areas of their well-being, not just their substance use, to achieve the best possible outcomes for every person.
- You will ensure that clients are supported to appointments, are building recovery capital and are aware of the support services across the city whilst working in a multi-disciplinary team to co-ordinate care and end the homelessness cycle.
- To engage with and support individuals experiencing Complex needs into treatment, working alongside various key partner agencies throughout the County. The role will require a large amount of multi-agency working to support clients into relevant treatment to further stabilise and move forward.
