**MAIN DUTIES & RESPONSIBILITIES**: The post will work both within the community and the hospice, providing support to children, their families and colleagues. The post holder will work alongside colleagues to provide 24-hour care, seven days a week. The post holder will sit on the hospice Senior Care Leadership Team. They will contribute to governance, including the writing of policies, procedures, audit and research and the delivering of training and education.

To provide line management to clinical nurse specialists. Clinical Responsibilities Undertake clinical assessment and to plan, deliver and evaluate holistic and evidence-based care to children and young people with life limiting conditions, as an integral part of the hospice interdisciplinary care team. Act as an independent prescriber within scope of professional practice, prescribing a range of medicines. Act as a point of contact for symptom control and palliative management advice for children and young people, in the full range of settings (home, hospital, hospice and school).

Maintain specialist level palliative care knowledge and skills across a range of procedures and practices including medicines management. Participate in the 24/7 on call service, working collaboratively with senior nursing colleagues to provide on call support to children on our caseload, in the hospice and community setting. Provide clinical advice and support to external professionals across the health, social care and education sectors, ensuring a coordinated multiagency approach to the provision of palliative care for children and young people. Develop, review and evaluate comprehensive symptom management plans (including CYPACPs), coordinating and supporting their implementation and use across agencies and Teams that children access.

Network at local, regional, and national level. Represent the hospice at the child death overview panels and share associated learning with the care team. Attend regular meetings as appropriate contributing positively to discussion and debates for shared decision-making. Care Lead Management requirements: Work as part of the hospice Senior Care Leadership Team to develop and embed a culture of innovation, personal development and continual service improvement within the care team and wider organisation.

Establish effective communication with relevant individuals and groups, including key members of the hospice, the locality community nursing teams, service users, other hospices and relevant statutory and voluntary organisations. To be an active member of the Hospice Clinical Governance Committee and allocated governance subcommittees Participate in the effective recruitment and retention of care staff, working closely with the Care Management Team and ensure appropriate support, supervision, and regular appraisal of hospice associate clinical nurse specialist. Lead on the development and effective implementation of organisational policies and guidelines as required. Education Provide clinical teaching within the hospice and to agencies we work alongside.

Empower and encourage staff to reach their potential. To work with the Care Leadership Team to identify areas of need within the hospice and support the learning of the Care Team to enable them to deliver a quality service. Personal Professional Development Actively participate in own regular appraisal process and be responsible for own professional development, including identification of individual needs and professional development needs, and plan to achieve these. Actively contribute to research and develop an independent research portfolio Comply with the NMC (Nursing and Midwifery Council) The Code (2018) Maintain a high level of professional standards to retain public confidence.

To actively engage in clinical supervision. Training and development Attend staff development programmes, including all mandatory training and eLearning as required. Take an active role in the staff appraisal scheme as an appraisee and to be responsible for setting and agreeing a personal development plan with your line manager. Develop and maintain an awareness of evidence based practice and current developments in childrens palliative care and the implications these may have for the delivery of care.
