The Learning from lives and deaths - People with a learning disability and autism (LeDeR) programme team are seeking Reviewers to join their bank.

Carry out high quality reviews in line with the standardised review process.

Act at all times in a professional, compassionate, and caring way, treating bereaved family members and loved ones with utmost respect and with courtesy.

Work collegiately and provide peer support and supervision to the team.

Share expertise and knowledge to support the review process.

Use professional judgement and knowledge Identify areas of improvement, gaps within services and areas of good practice.

Escalate any concerns to the Senior Reviewer and Local Area Coordinator.

The primary role is to complete reviews, interpreting complex and highly sensitive information into a concise and readable manner, providing services with an opportunity to understand areas of improvement, gaps or areas of good practice. The review process can includes talking to professionals and families in a respectful and sensitive manner, developing a shared understanding of the person and their situation, and recording this as part of the review. Reviews should be carried out in accordance with the standardised review process and to the highest standards, focusing on service improvement.

Consider the best evidence available including the conversation with the family or loved one, conversation with the general practitioner (GP) or GP records and other health and care data sources to inform the review.

Use professional judgement and experience to determine and agree with senior reviewer, whether a review proceeds to a focused review.

Present review findings at governance panels and support the panel to define specific, measurable, achievable, realistic, timely (SMART) actions offering professional challenge to ensure that the actions that are developed will achieve the needs of the findings of the review.

Link with, escalate and refer concerns where necessary and advise families where another process (for example a Trust complaints process) would be more appropriate.

Complete the relevant training initially and then the refresher training on an annual basis.
