**Job overview**:
The Health Visiting Service is part of the children’s integrated community health services directorate whereby services deliver an integrated and holistic model of care to meet the needs of children, young people and families using the Rochdale Needs and Response framework model.

The HV CONI Specialist will use their advanced clinical skills to provide leadership and operational management for the provision of specialist support to parents with a subsequent birth, following an unexplained death of a baby. The Specialist Practitioner will work across HMR footprint to promote equality in provision via CONI link HV’s. The Specialist Practitioner will raise awareness of the CONI programme with key stakeholders and in partnership with HV manager develop local structures for the provision of resource and the collection of data.

**Main duties of the job**:

- Using specialist knowledge and evidence-based practice to ensure
appropriate delivery, evaluation and audit of interventions.
- Work in partnership with Midwifery colleagues and regional networks to monitor local, regional and national Sudden Unexplained Deaths of Infants (SUID) and indicator of risk.
- Provide leadership support for the Team Leader in the day-to-day
management and co-ordination of a team of Health Visitors and skill mix staff, within a defined neighbourhood.
- The post holder will have continuous responsibility for the assessment, planning, implementation, and evaluation of the health needs of a defined population.
- Work in partnership with individuals, families and communities and collaborate with the wider multi-agency team in accordance with the local and national public health agenda.
- Evaluate and develop systems to identify mothers booking for antenatal care with a previous history on SUDI
- To undertake any other reasonable duty, which is appropriate to the band, when requested by senior staff
- To be familiar with and comply with all Trust and departmental policies, procedures, protocols and guidelines
- To be aware of and work towards the Trusts strategic goal
