**Care Coordinator (maternity cover 12 months)**

Cricket Green Medial Practice is a forward-thinking, well established GP surgery in South West London. We provide care for a patient population of close to 12,000, working alongside a multi-skilled team of 40 clinical and administrative staff, supported by four GP partners.

We have a temporary opportunity for a Care Coordinator to cover a period of maternity leave, with a period of handover depending on start date. You will be working with our highly supportive and skilled team, manning the reception desk in a very busy surgery. This role also involves administrative tasks; experience working in a similar environment, such as a GP, NHS, dental or veterinary practice, or in a similarly fast-paced customer-facing environment, will be an advantage.

**We need you to work 28.5 hours per week across 4 days: Mondays, Tuesdays, Wednesdays and Fridays. **Specific hours are shown below - you must be available to work those times.

There may be an opportunity to increase hours to 30 per week by working longer on a Wednesday. Overtime will be available to cover staff absences and flexibility will be appreciated.

**About the role**

You can expect a very busy but varied day, with a structured rotation of tasks that cover patient-facing reception duties, as well as admin tasks. Key aspects of the role include:

- Being the first point contact with patients in the GP practice on the front desk
- Answering phone calls from patients, pharmacies and care homes and signposting patients to the best service
- Inputting patient requests into our triage platform
- Booking and cancelling patient appointments with the nurses, GPs or other clinicians
- Process prescription requests
- Send /book patient referrals
- Scanning and filing incoming letters from hospitals and other services, adding information (medical codes) onto the clinical record and actioning requests from the hospital if required
- Responding to enquiries from patients and other healthcare services

**About you**
- You will have excellent customer service skills and the patience to deal with a wide mix of patients and enquiries.
- Your proven organisation skills and ability to prioritise, and willingness to muck in to serve our patients and help your colleagues, will serve you well in this fast-paced role.
- We are looking for someone who is willing to learn and ask questions, in order to provide accurate and helpful information to our patients at all times.
- You will have a good, solid knowledge of Word and Excel, and the ability to pick up new systems quickly (with training).

**What you can expect**

1. A fixed shift pattern, as shown below:

- Mondays: 10:30 to 18:30
- Tuesdays: 08:00 to 16:00
- Wednesdays: 12:30 to 18:30 (or 10:30 to 18:30 if you want 30 hrs per week)
- Thursdays: n/a
- Fridays: 08:00 to 16:00

2. A rate of pay of £13.15 per hour - for 28.5 hours per week, that’s £19,488 per annum (or £20,514 per annum if working 30 hours per week)

3. Four weeks annual leave, plus the bank holidays (pro rata for part-time working)

4. A 6-month probationary period (or 4 months if contract is for less than 12 months)
