**We are looking for a Reablement Officer to work for a local council within East London.**

To provide intensive, short term support to enable people with physical or mental health needs to enable them to lead a fulfilling life, by learning or re-learning skills for independent daily living.

**£18.76 per hour PAYE**

**Full Office Based within Newham**

**MUST hold a qualification within Social Work**

To plan and assist the Service Manager, Senior Physiotherapist or Senior Occupational Therapist and other managers, in managing the delivery of all required rehabilitative services within their assigned area, in a competent and professional manner, ensuringcompliance with social care Reablement objectives, which are:

- To provide Reablement service to all appropriate people in their home
- To provide an initial assessment of need and Reablement potential
- To establish goals and required outcomes in agreement with the individual concerned
- To monitor reablement goals being delivered by third sector providers ensuring that the citizens of the local council have access to high quality services which safeguard and promotes their health & well being.
- To provide regular reviews of progress towards outcomes
- To provide appropriate support towards achieving outcomes
- To liaise and work collaboratively with other health, social and third sector services to meet people's needs
- To recommend and/or arrange on-going support/advice/sign-posting for individuals on discharge from the service, as appropriate
- To provide equipment and assisting technology to meet assessed needs

To assess individuals to determine their Reablement and on-going support needs, through self-directed support, to develop rehab support plans in consultation with the individuals and monitor progress made.

During the Reablement process, to carry out social care assessments and light touch reviews as required, and to hold case management responsibility
- To maintain effective and positive communication with management, colleagues, customers and other external agencies.
- To ensure continuity of care provision through effective contingency plans.
- In addition to the Adults Reablement Service, the post holder may manage other care service elements. This includes telecare.
