Rapid Response Care Assistant

**Job description**
As a Rapid Response Care Worker for Stayathome Cornwall, you’ll play an important role in providing intervention to help with hospital discharges across Cornwall. On a typical day, you will visit immediate hospital discharges at home, completing their care visits involving personal care, for a short period until they move on to more permanent care. As part of your role, you will be required to contact relevant professionals, fill out paperwork and relay information to lead assessors. We have roles available in various areas in Cornwall with shifts on rotation from Monday - Sunday.
**About You**
- NVQ Level 3 in Health and Social Care
- Full, clean driving license and own transport
- Great communication skills

**Job Types**: Full-time, Part-time
Pay: £12.50-£13.00 per hour
Expected hours: 16 - 48 per week
