Sheffield County have a great new opportunity to join a brand new service. It is a 12 month service where we are being commissioned to test and learn around hospital discharge, community connections and frequent A&E attenders. This role will work alongside other services to identify when Service Users are to be discharged from hospital with no fixed abode.

**Responsibilities**:

- Identifying and supporting those as early as possible to prepare for hospital discharge.
- Liaise closely and facilitate effective communication and cooperation with The Sheffield City Hospitals and A&E teams.
- Work closely and develop relationships with the hospital social work and discharge teams.
- Liaising with Sheffield City Council, with social care, voluntary, criminal justice, and substance misuse agencies.
- Liaising with, social and private landlords and other agencies to identify those that have no fixed abode, are at risk of rough sleeping.
- Aim to improve communication, connectivity and awareness between hospital teams and community services.
- Identifying other services already involved with the identified cohort and making the links with them to support individuals to utilise existing and developing primary care/community resources as an alternative to the default position of A & E
- Take a lead in coordinating an appropriate package of support for the service user.
- Help contribute to system change and develop new pathways to bolster the homelessness prevention agenda.
- Performing initial assessment, offering ongoing support, and developing self-management plans on a range of issues.

Desirable Experience
- Experience of working with people with multiple and complex needs and supporting individuals to make a positive change.
- Understanding of the issues that people rough sleeping face
- Numeracy and Literacy Skills equivalent to English and Maths GCSE A-C or equivalent qualification
- Knowledge of local adult and child safeguarding procedures.
- Knowledge of local services and potential referring groups
- Manages caseload and workload effectively and efficiently
- Full driving licence and vehicle available for work
- A requirement of the role is that you work as part of rota system including unsociable hours.
