We have an exciting development opportunity for an experienced administrator within our Ageing Well MDT Coordinator Team. This secondment or fixed term position also offers hybrid working.

You will be working within a highly supportive team of 5 administrators, making a difference by supporting patients utilising a Community Multi-Disciplinary Team (MDT) approach.

As an AW MDT Co-ordinator you will need to be able to work on your own initiative and manage your own workload. You will access different systems to gather patient information to present at AW MDT meetings, providing the multi-professionals team an overview of a patients social situation and needs. Being part of the AW MDT meeting enables different services to collaboratively discuss the patients needs and how to support not only the patient, but also the carers and family.

This is a rewarding role, as you see first hand the benefits an Ageing Well MDT meeting can bring to a patient and their family.
