Job summary

An exciting opportunity has arisenfor a Physiotherapist for the Community Neurology Rehabilitation Team based in MountGould Hospital. We are seeking a candidate who is able to operate withinvarious community setting and provide advanced assessments of patients withneurological presentations.

Main duties of the job

The post holder will be workingwithin a multi-disciplinary team to deliver individualised treatment programmesthrough advanced assessments and undertake all aspects of clinical duties as anautonomous practitioner. This will include utilising investigative andanalytical skills to formulate individualised management and treatment plans, usinga range of communication to communicate with carers to gain their co-operationin order to progress patients rehabilitation and to operate as a lonepractitioner when working in community and domiciliary settings with support ofmanagement via telephone. There will be a responsibility to provide supervisionto junior colleagues, plus deliver expert advice, teaching and training to othermembers within the MDT regarding the management of neurological patients whilstworking jointly with colleagues in a co-ordinated multi-disciplinary approach.

Livewell Southwest provides integratedhealth and social care services for patients across Plymouth, South Hams andWest Devon, as well as parts of Devon and Cornwall. By integrating health andsocial care we are able to deliver the right care for people, in the rightplace and at the right time. You will find our teams across the community inhospitals, GP practices, sports centres, health, and wellbeing hubs.

To be successful in applying for this role you will need:

Currentrelevant professional registration Minimum 2years post-qualification experience Enhanced DBS UK/NHSexperience Experience of workingin a community setting Experience ofworking within Neurology Full UKlicense and access to own car for work purposes
