The role of Health Care Assistant is in place to provide end of life care to patients in their own homes in the final weeks of life. You will be working with a qualified nurse or paramedic responding to patient needs as they arise. A car and driving licence will be essential.

**This means you will be responsible for**:

- Competently working along, or with a registered health professional (nurse or paramedic), as required, with patients and their families in the home environment with support and guidance from qualified professionals.
- Respond to all calls and request for patient visits, speaking with patients and relatives as required.
- To adhere to the lone worker policy and procedure.
- Working collaboratively with hospice and community partners.
- Acting as a Rennie Grove and Peace ambassador within the community
