Emergency Medicine is evolving rapidly. We treat 130,000 patients a year from Code Red polytrauma patients brought in by our HEMS doctors to patients with minor injuries. Our close links with Brighton and Sussex Medical School means you can also inspire new generations of doctors with the excitement of what we do.

The year-long posts (FT or LTFT) allocate 25% non-clinical time for projects within the Emergency Department (ED). Future projects are customizable based on individual career needs, interests, and training development, discussed with the ED Clinical Fellow program lead.

**Examples of past projects include**:
**Education**: Supervising and teaching medical students in the ED and at the Medical School. Eligibility for a PG Cert in Medical Education.

**Simulation**: Conducting in-situ multidisciplinary simulations for trauma calls and emergency scenarios. Presentation at national and international conferences.

**Trauma**: Audit presentations, contribution to MTC governance, involvement in ED trauma research.

**Leadership/Quality Improvement**: Establishing community pathways, updating guidelines, implementing digital services, teaching on leadership.

**Ultrasound**: Personal development, multidisciplinary teaching, developing online resources.

**Research**: Participation in clinical research, academic supervision, presentation, and publication opportunities.

In addition, the rotas create efficient use of staffing so that there are not the fluctuations in numbers that happens with traditional templating systems. The three actually go hand in hand because we believe that if you improve staff welfare and job satisfaction then you improve patient care, all whilst improving efficiency.

The rota works via a fundamentally different way of staffing doctors - via an annualisation system. Full allowance of annual leave, study leave and bank holidays (whether worked on bank holidays or not) are allowed for in the annualisation calculations. This calculates a set number of clinical shifts, which have to be worked over the year. This allows for greater flexibility than standard rosters. Weekend frequency is no more than 1 in 3 weekends but with compensatory time off during the week.

The full time jobs are 48 hours. 3 hours non-rostered time is included for work to include audit, quality improvement projects as well as traditional teaching.

These jobs are designed to supportpA&E reg doctors to do clinical work but with non-clinical paid time to facilitate job sustainability and satisfaction. It is great for those wishing to take some time out after FY2 who want flexibility, stability, training, supervision, a study budget as well as GMC revalidation but do not yet want to commit to a training programme. They are also great for people who want to take some time out of a formal training programme where they have dedicated non-clinical time.

The rota and the clinical fellow jobs in A&E have won recent RCEM quality improvement prizes and a HSJ highly commended prize in the workforce category and were shortlisted for the BMJ innovation prize. They are used by exemplars by NHS Improvement, HEE (Health Education England) and Deloitte for solving the NHS staffing crisis.

BSUH was awarded the first place Emergency Training Department of the Year by the Royal College of Emergency Medicine in 2018.

The Health Service Journal (HSJ) awarded BSUH this initiative the ‘Acute Services Redesign’ winner for 2018. Registrars do not do single clerking but reviews, and traditional clerking.
