**Job Title**:Home Support Coordinator**

**Team: Reablement - Home Support**

**Hours: 30 per week**

**Grade: F SCP 17, £28,770 to SCP 23 £32,076 (7% enhancement weekend working)**

**Pro rota 30hrs is £23,327. Pro rota 18hrs is £13,996.**

**Contract: Permanent**

**What do we do?**

The service provides a stepping stone to independence, by supporting people to regain lost skills, learn new skills, and adapt to the challenges that living independently can present.

You will work within a CQC “Good” rated Home Support Reablement Council Team, and will have an opportunity to work with other professionals such as our colleagues in the NHS, Social Workers, Occupational Therapists etc to provide all aspects of care, which includes but not exhaustive of:

- Moving and Handling
- Personal Care
- Administering medication
- Ensuring service users dignity and wishes are at the centre of the care provided.

Home Support Reablement is a short-term service, meaning there is a quick turn over of service users.

**What is the role?**

The role of a Co-ordinator is to support people in their own home who are at risk of needing social care or have an increased intensity of care.

You will be:

- reviewing care plans and risk assessments,
- managing staff, including completing Direct Observations and Supervisions,
- prioritising and managing your own workload,
- ensure staffing levels are safe for service users and staff,
- screening referrals into the service,
- allocation of workload to staff,
- signposting to external services,
- attend staff meetings and support worker meetings,
- ensuring CQC compliance.

**What will your working week look like?**
- 30-hour contracts available
- Rolling rota
- Variety of early, lates and weekend shifts

**Job Description**:
**About You**

Do you have
- a Full Driving License and access to a vehicle to use for work purposes?
- a passion to support and care for adults in the community?
- the ability to work on your own initiative?
- the ability to communicate clearly with people of all levels?
- the skills to work in the community to encourage people to engage and reach their potential?
- experience of delivering care in a Health and Social Care setting?
- experience in supporting or mentoring staff?
- knowledge and experience of IT systems?

A comprehensive induction and training programme will be provided on succession into the role.

You must ensure that the service is delivered to the highest achievable professional standards to support vulnerable adults with both rehabilitative and / or mental health needs to retain community living.

Your work will be compliant with all CQC regulations, and Bolton Council policies and procedures.
