We are recruiting for a Care Team Leader in Dalton-Le-Dale, Durham to support people to live in and be part of the local community, within a supported living environment. Our services are about helping people to get out in the community and make the most of time in their own homes.

**Position**:Care Team Leader (Supported Living)

**Location**:Dalton-Le-Dale, Durham

**Hours**:40-48 hours per week, a mixture of day and night shifts

**Rate of pay**:£11.80 per hour with an additional £1 enhancement per hour for this service

**About our service**

We support people with a range of support needs including learning disabilities, autism, mental health support needs, acquired brain injuries and other complex care needs, we empower each person to live life the way they choose to.

This service in particular is supporting someone with their mental health and emotional well being whilst supporting to help maintain independence and their own tenancy.

**About you**
- **Are you enthusiastic, compassionate and committed to supporting people in a way which respects their dignity?**_

You will need to be adaptable, flexible, with excellent communication skills and able to work as part of our team. Although not essential it is desirable to:

- Have previous experience supporting people with learning disabilities / mental health, although all training will be provided
- Hold or be working towards a Level 3 in Health & Social Care (or equivalent)
- Hold a valid driving licence

**The role**

Provide practical assistance and emotional support to empower the people we support to achieve their goals and aspirations, respecting everyone’s individuality and enabling them to make appropriate choices to live the life they wish, in line with their individual support plans. Including but not limited to:

- Promote the health, well-being, physical, emotional, cultural and spiritual beliefs of the people we support and ensure they are met in an inclusive way.
- Facilitating social, educational or work activities such as visiting family and friends, walks, shopping and other recreational activities of their choice.
- General housekeeping duties that can include shopping, domestic duties as well as elements of personal care.
- As part of the role, you will need to be able to complete a range of records and documents.
