An exciting opportunity has arisen to join Suffolk County Council (SCC)** East Learning Disability and Autism Team** as an **Independence and Wellbeing Practitioner,** working from Stowmarket.

**Your role and responsibilities**

The main purpose of your role will be to work with customers to identify and deliver personalised good quality outcomes to the highest standards to support and maximise their independence and wellbeing.

You will:

- work as part of an area social work team alongside colleagues and practitioners across the Health and Voluntary sector
- support effective integration between health and social care
- ensure systems are joined up and accessible for all our customers
- promote Suffolk County Council’s vision, objectives, and priorities effectively to your customers, our partners, and the public
- inspire others by role modelling our organisational values at all times
- promote a culture of value for money, outcome-based customer focus and continuous improvement.

**You will need**
- Good standard of education.
- Experience of working with people with disabilities.

**The team**

This is an exciting time to join our Learning Disabilities and Autism teams within Adult and Community Services and improve outcomes for people.
