*Additional Information: *This position will involve travel to the homes and services throughout the week, working hours are Monday - Friday and working from the homes and services is required.

*Job Description but not limited to: *
- As guided by the Clinical Lead and the Senior Practitioner to undertake assessment, formulation work alongside individuals in the context of their history and relationships.
- To co-ordinate on going behaviour monitoring of challenging behaviours.
- To assist in the development of interventions aimed to reduce the intensity, frequency and potential challenge of behaviours that are challenging to the individual and the service.
- To work with members of support staff to promote independence and the other outcomes as identified in in Living the Life profiles or Recovery Star.
- To facilitate the smooth and efficient implementation of the Care Plan systems within the homes.
- To attend reviews, staff meetings, key group meetings, service user meetings etc., as appropriate.
- To attend bi-monthly meetings in the services with the Clinical Lead, Senior Practitioner and the manager to review each service user.
- To provide written reports of reviews and other assessment, formulation and intervention work.
- To maintain a written report of any individual work.
- To undertake training for trainers if required to do so.
- To liaise with staff working at the homes on matters relating to people with learning disability and/ or mental health difficulties.
- To participate in the staff training programme.

*Where would I work?*

Predominantly within the homes and services however working from the Bracknell Head office is also expected from time to time.
