**Job Title**: Young Adult’s Specialist Practitioner (YASP)

**Salary**:£21,500 per annum

**Expenses**:Staff may claim travel expenses, at the agreed rate, for journeys which are necessary in the course of their work. Claims for additional expenses will be reimbursed in accordance with our expense policy.

**Annual Leave**:28 days per annum, excluding bank holidays

**Role Outline**

This is an innovative new service, integrating the expertise, skills and knowledge of the VCS (Voluntary and Community Sector) network in Barnet with NHS Mental Health services. The purpose of the role is to provide psycho-social support to clients aged 18-25, working within specialist Mental Health teams and the partners of the Wellbeing Together CIC.

The role will carry an active caseload supporting access into services appropriate to their level of need, with a focus on recovery and independendence, and improve self-management of their wellbeing through intensive support. The YASP will support the core community mental health teams working in alignment with Primary Care Networks as part of the new model.

The YSAP will focus on empowering clients to achieve socio-economic goals with clear mental health and wellbeing benefits, and team members will also have specific training in areas such as employment and housing to support the wider PSS service team. The post holder will help to faciltitate and co-deliver recovery focused groups and contribute to the development of new community resources.

**JOB SUMMARY**
- Build a therapeutic relationship with clients who are transitioning from CAMHS services and young people accessing services for the first time using a person-centred approach/social model of care.
- Work collaboratively as part of the multidisciplinary team with clients to develop their transition plan and promote personal responsibility.
- Advocate the views and interest of clients, when requested, while accessing other local community and statutory services.
- Use your lived experience in all appropriate situations, including to build rapports with clients and address the stigma of mental health challenges with colleagues.
- Act as a mentor and role model to empower beneficiaries with the ability to have a meaningful life for clients.
- Build relationships with the local community and statutory services and connect clients to those resources.
- Input client notes and progress on the Trust’s electronic patient records system.
- Record client clinical information on the Barnet, Enfield and Haringey (BEH) Trust’s electronic patient records system (RIO).

**Client Centred Support**
- To establish a supportive and respectful relationship with clients.
- Work with clients to ensure a clear transition plan is in place.
- Arrange transition meetings and joint sessions with Children and Young People/Younger Adults and their clinical teams.
- Act as named key worker for Children and Young People transitioning to Core Community Localities and other MH services.
- Oversee the production of personal plans and hospital passports.
- Work with clients to help them identify their own meaningful recovery and personal goals drawing on your mutual resources as peers and utilising a range of recovery tools, techniques and experience.
- Encourage and motivate service users to create co-produced care plans and enable clients to engage with the actions of their care plan.
- Accompany service users, if appropriate, to appointments/meetings where they need additional support.
