We have an exciting opportunity for a highly motivated, enthusiastic Phlebotomist, to work with the Integrated Community Team in the Chesterfield and Dronfield locality.

The post will be mainly based at Walton but will cover areas across both central and east Chesterfield. When required you may also be asked to cover parts of Dronfield.

The post involves working 20 hours per week over either 4 or 5 mornings, with Friday being essential.

Working hours can be flexible to applicants needs. This post does not require to work any weekends.

The applicant will need access to a vehicle and a full driving Licence for this post.

Our team focusses on providing excellent patient-centred care to people within their own homes. Therefore, it is essential that you have a keen interest and experience in this field.

If you can demonstrate good communication skills, have the ability to plan and organise competent, evidence-based, clinical care delivery - we want to hear from you

Our purpose is to provide community health services to a patient population of over one million people in Derbyshire and Derby, as part of Joined Up Care Derbyshire.

We employ around 4,200 substantive staff, caring for patients in 11 community hospitals and more than 30 health centres, as well as in clinics, GP practices, schools, care homes and, increasingly, in people’s own homes and via virtual consultations.

During the pandemic response we have led the implementation of public vaccination centres in collaboration with partners across health and social care.

The duties of the post, under the direction of the Integrated Community Team, are to:

- Carry out venepuncture and other appropriate healthcare procedures in patients’ own home or in community health care settings, acting within the procedures outlined in the Phlebotomy competency framework
- Undertake appropriate patient care assessments as required, such as baseline observations
- Escalate any concerns regarding patients’ welfare to the registered members of the Integrated Community Team in a timely manner
- Once a visit is complete, enter contemporaneous notes in the relevant patient information system
- Use DCHS patient information systems as needed, including accepting e-referrals
- Recognise the importance of confidentiality and sensitivity of issues at all times, and be able to act using the utmost discretion and confidentiality
- Additional duties and ad hoc requests may be required within this role and the need to be flexible is essential
