The Crisis Response and Home Treatment Team is a key part of the Urgent Care Pathway meeting the demands of the Ten-Year Plan to provide Crisis Response and Home Treatment Services by 2021. Clinicians employed into the team are part of an exciting and developing service aimed at supporting our most vulnerable patients and their families in the least restrictive setting possible by providing a variety of highly intensive interventions at home.

We require RMNs/clinicians who can bring their own professional expertise to a fast-paced team, and who are confident in managing risk in the least restrictive way. By its nature, crisis work requires professionals who have a broad range of skills including being skilled mental health assessors with a proven ability to signpost service users towards the most effective treatment pathways.

Crisis can manifest with a range of needs and whether you have a background in mental health nursing, social care or occupational therapy we want you to use your skills as part of a team to develop individualised care packages for our service users and their families.

Service Users access a crisis response in a variety of ways, ranging from referrals from their GP and attendance at an emergency department, to when they need care in the community and are seen by the police and our street triage teams. The aim of the Urgent Care Pathway is to bring together all those access points under one team, including:

- Psychiatric Liaison
- Overnight Response Team
- Assessment Teams
- Street Triage and Health Based Places of Safety

The successful applicants will participate in a duty rota of early, late or long day shifts covering 7 days a week and we are committed to supporting staff to balance home and work life. We are a friendly and supportive team with strong leadership and direction. Our work is interesting and varied with plenty of opportunities to learn new skills both in the workplace and via our excellent Learning & Development team. The role is predominantly based in Aylesbury and may require some wider travel within the county.

We would welcome clinicians who are keen to develop or further enhance their skills in a variety of areas and to help shape this exciting and developing service.
- To triage new referrals into the team with our consultant psychiatrist and to manage them with an appropriate level of urgency in relation to the presenting risk.
- To carry out the initial assessments of service users who meet the referral criteria of the CRHTT and to undertake short term treatment where appropriate.
- To manage, formulate and implement treatment plans, delivering care under the clinical model.
- To evaluate effectiveness whilst working within the framework of CPA and Risk Assessment.
- To report all relevant information to the referring agent and keep them informed of the service

users progress.
- To assess, plan, set goals with the service user, carers and families; implement and evaluate care

using psycho-social interventions and skills to meet individual needs.
- To deliver individual therapeutic activity using therapeutic interventions and skills.
- To be responsible for monitoring and evaluating treatment plans, transferring service users to the treatment function, other teams within the Trust, or discharging them when appropriate.
- To liaise and establish good working relationships with referring agents, colleagues in Primary Care services, Social Services, Community Mental Health Teams, Assertive Outreach, In-Patient services, Criminal Justice, Voluntary Sector and other agencies as appropriate.
- To be an active member of the multi-disciplinary team and to attend professionals’ meetings and other work led meetings as required
