An exciting opportunity has arisen in Hywel Dda University Health Board to join our front of house team, TOCALS (Transfer of Care Advice and Liaison Service). This is an innovative frailty focused team in place to preventunnecessary admissions into acute care, with an understanding of the risks that such admissions can cause for patients. The post will involve working within Glangwili Hospital's front of house services, including Accident and Emergency, Same Day Emergency Care, Minor Injuries Unit, and the Clinical Decision Unit.

The post holder will be workingas part of a multi-disciplinary team including Physiotherapists, Occupational Therapists, Social Workers, Discharge Liaison Nurses, and Delta Workers.

Applicants will need to be highly motivated with a keen drive to achieve patient centred, holistic, and compassionate care. With a wide variety of patients and conditions seen within front of house services, and through working alongside a wide array of multidisciplinary staff this post brings with it a wealth of learning opportunities. Dueto the high pace of the environments, applicants will need to be adaptable and able to work in busy and pressurised environments.

Duties will include supporting and completing holistic patient assessments, with consideration of strategies/options to aid a safe and timely discharge.

The post holder will play a pivotal role in delivering rehabilitation to patients admitted to the Clinical Decision Unit to promote each patient's independence. This could include bed, chair, and standing activities, gait re-education, and gym based exercise programmes.

You will be expected to support patients at times alone, though all work undertaken will be under the guidance of qualified Physiotherapists. Recognition of your scope of practice is of paramount importance to recognise when qualified support is required.

To facilitate continuity of patient care, this post will involve a degree of in-reaching into acute ward areas, and out-reaching into the community.

The University Health Board proactively supports training and development both as part of internal and external CPD and you will be expected to complete appropriate mandatory training.

48 general practices (four of which are managed practices), 47 dental practices (including three orthodontic), 99 community pharmacies, 44 general ophthalmic practices (43 providing Eye Health Examination Wales and 34 low vision services) and 17 domiciliary only providers and 11 health centres;
Numerous locations providing mental health and learning disabilities services;
Highly specialised and tertiary services commissioned by the Welsh Health Specialised Services Committee, a joint committee representing seven health boards across Wales.
