We are seeking therapy assistant practitioners to join our rapidly developing stroke service in Carmarthenshire, supporting early discharge from hospital.

If you have a passion for rehabilitation working across the stroke pathway this could be the opportunity for you. Part of a new Health Board wide Therapy Early Supported Discharge Service, you will be based in the County of Carmarthenshire, supporting people who have predominantly experienced a stroke to return home from our acute stroke units.

There are 3 posts available - bases are yet to be confirmed within Carmarthenshire.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

The post holder will be required to participate in a 7 day service as required.

The role will aim to deliver rehabilitation at home and in the local community to support transition from hospital and return to independence. To deliver a person-centred rehabilitation and transition pathway the post holder will work alongside the established multi-disciplinary teams to ensure good continuity of care.

Successful applicants will have some experience of working with people who have experienced stroke and/or other life changing events and enjoy working as part of a multi professional team within a community setting. You will work alongside acute and community colleagues and services to create a joined up rehabilitation pathway and positive experience for our population. Whilst the role focusses on stroke, we require therapists who are adaptable to meet needs of patients with multiple and a variety of conditions as well as service priorities. Due to the nature of the role you will need to be able to travel on daily basis within the County, as well as being flexible to travel across the Health Board on occasions.

You will work under supervision of the qualified therapists to provide an efficient, caring, patient focused rehabilitation.

The University Health Board proactively supports training and development both as part of internal and external CPD and you will be expected to complete appropriate mandatory training.

Hywel Dda University Health Board is the planner and provider of NHS healthcare services for people in Carmarthenshire, Ceredigion, Pembrokeshire and its bordering counties. Our 11,000 members of staff provide primary, community, in-hospital, mental health and learning disabilities services for around 384,000 people across a quarter of the landmass of Wales. We do this in partnership with our three local authorities and public, private and third sector colleagues, including our volunteers, through:
**Four main hospitals**: Bronglais General in Aberystwyth, Glangwili General in Carmarthen, Prince Philip in Llanelli and Withybush General in Haverfordwest;
**Seven community hospitals**: Amman Valley and Llandovery in Carmarthenshire; Tregaron, Aberaeron and Cardigan in Ceredigion; and Tenby and South Pembrokeshire Hospital Health and Social Care Resource Centre in Pembrokeshire;
48 general practices (four of which are managed practices), 47 dental practices (including three orthodontic), 99 community pharmacies, 44 general ophthalmic practices (43 providing Eye Health Examination Wales and 34 low vision services) and 17 domiciliary only providers and 11 health centres;
Numerous locations providing mental health and learning disabilities services;
Highly specialised and tertiary services commissioned by the Welsh Health Specialised Services Committee, a joint committee representing seven health boards across Wales.

Access to lease / pool cars can be reviewed by the service. Travel expenses can be claimed as per approval by the post holders line manager.
