The CCRT is a forward-thinking, dynamic, developing and enthusiastic team providing evidence based Respiratory care to COPD patients. The Countywide Community Respiratory Service is well established and provides an Early Assisted Discharge Service, Acute Respiratory Assessment Service, Community Oxygen Assessment Service, Complex Case Management, Pulmonary Rehabilitation and Respiratory Physiotherapy across Lincolnshire. The services are currently managed in the 4 quadrants of the county, but the countywide respiratory team continues to work collaboratively and supportively across the quadrants. The team actively integrates and collaborates with neighbourhood teams throughout the county and liaises with all key stakeholders to provide seamless care. The CCRT also have excellent links with the University of Lincoln and are actively engaged in research, continually seeking opportunities to enhance the service specification of the team.

Lincolnshire Community Health Service requires all staff to

The post holder will participate as a member of the multidisciplinary team in the assessment, planning, delivery and evaluation of patients/clients health and wellbeing needs and participate in the delivery of complex care as prescribed by the qualified practitioner.
To organise and run exercise classes in the community and on
virtual platforms with the registered staff, maintaining high
professional standards. To be able to use identified assessment tools and outcome measures in line with agreed competency.
To have knowledge of specific physical, emotional, psychological, social, cultural and spiritual needs of the service users and carers and, taking these into account, assist and implement appropriate action to meet those needs.
Plan and progress patient interventions with autonomous assessing and planning on a daily basis with indirect supervision of qualified practitioner. Understand a range of non-routine work procedures and practises which require intermediate level of theoretical knowledge and physical skills gained through relevant training and experience.
Teach patients/carers with the indirect supervision of a qualified practitioner, and demonstrates educative support to others, e.g. participate in induction of new staff.
Demonstrate working knowledge of other provider’s resource
and referral systems to ensure patient needs are met, within
scope of practice.
To be actively involved in team effectiveness by attending staff meetings and contributing as a team player to team development. Contribute to the non-clinical day to day running of the service as necessary to ensure its smooth running.
Promote a positive image of the organisation, department and
service. Work flexibly across different locations in line with service demands.
