This is an exciting opportunity to work alongside our dedicated Adferiad team working in the provision of housing support which includes both shared accommodation and floating support for People with secondary mental health diagnosis.

Adferiad’s housing service provides a temporary supported move on accommodation project and the support is delivered either in the Core House (shared accommodation) & to individuals who live in the Cluster (Outreach) Flats. The Core House is a shared supported accommodation up to four occupants who are diagnosed as experiencing a serious mental illness and have been assessed as needing moderate levels of support to help with their recovery. In addition, Adferiad provides individuals support who have their own individual occupancies, with each person receiving their support from Adferiad staff to maintain their tenancy & independence.

Under the direction of the Service Manager, work as part of team or lone working to support, encourage and enable clients to reach a greater level of independence over a reasonable period of time, and then to facilitate their move on to more independent living in the community.

The service operates 7 days per week, with a shift pattern, including weekends, unsocial hours. Shifts will include staff lone working.

**Main Duties and Responsibilities**:
All Adferiad Support Workers will be given the qualifications, experience and competencies commensurate with the levels of care they are expected to provide and in accordance with the Care Inspectorate legislation for Domiciliary Support & Housing Related support.
- Provide a variety of person centred, tailored interventions that support individuals with complex needs to find and sustain accommodation and develop their independent living skills.
- Supporting and assisting occupants with the following:

- Increase independent living skills.
- Mental and physical healthcare.
- Building and maintaining relationships with neighbours, families, friends, professionals.
- Pursuing training/educational/employment/leisure & recreational goals and aspirations.
- Signposting, joint work and referral on to other specialist agencies, intervening in crisis situations.
- Developing links with the local community-based resources and opportunities.
- Recognition of and coping strategies in respect of risk, triggers and avoiding relapse.
- Work with each individual to achieve their aspirations and the goals of the project, employing creative and innovative methods to meet these outcomes and, through staff’s own experience and resources, bring added value to the support role.
- Working with individuals in a timely and task specific manner on a variety of issues in order to impact positively on issues relating to housing, employment, training and education and social isolation.
- Liaise with relevant services, organisations and agencies with regards to accessing additional support for service users.
- Practice homeless prevention, tenancy sustainment, Psychologically Informed Approaches, Solution Focus Practice, harm reduction, and co-production.
- To provide a service to individuals, that complements and enhances the service provision of community services, local housing providers and other agencies.
- Support service users to exercise informed choices about their lives where possible and to enhance their social skills and participate in their community.
- Support the Service Manager & Senior to ensure that individual needs assessments, risk assessments, support plans are reviewed regularly and are up to date.
- To carry out all other tasks commensurate with the role.

**Person Specification -**
- No specific qualifications are required however NVQ competencies in Health & Social Care Level 2 would demonstrate appropriate competence for the post
- Full driving licence and full access to a vehicle for work purposes
- A sound knowledge of safeguarding adults, children and the protection of vulnerable adults
- Knowledge of, and ability to work with, issues of confidentiality, consent, and capacity
