If so, an exciting opportunity has arisen within our Heart/Lung Transplant and LVAD team at the Queen Elizabeth Hospital Birmingham.

We have a permanent position available.

We are passionate and proud about delivering excellent patient care. Our service looks after patients throughout their whole pathway from assessment for transplant to post transplant/LVAD care for life. This is both in the in and outpatient setting.

We are looking for someone who is suitably qualified and experienced within cardiothoracic. Ongoing training, development and support will be offered.

The successful applicant will have a motivation and determination to succeed; be able to demonstrate sound clinical knowledge with a special interest in transplantation and mechanical support, they should have the ability to work autonomously and within the MDT. You will need to be flexible and resourceful. We offer a 24hour care to our patients including an on call service.

**Main duties, tasks & skills required**:
The post holder will work in conjunction with members of the multidisciplinary Heart and Lung Transplant/VAD (left ventricular assist device) team to coordinate the nursing and managerial aspects of the patient pathway, pre and post heart and lung transplantation and mechanical circulatory support insertion. These will include:
Provide expert communication, education, support and advice to patients, families, colleagues and

other health professionals in the inpatient, outpatient or community settings within a 24 hours

service.

Coordinate the whole patient pathway for patients pre and post heart and lung transplantation

or mechanical circulatory support, including admission to and discharge from hospital for

assessment, procedures and post-operative care

Receive and act upon all National and European cardiothoracic organ offers under the supervision

of Mentor and Consultant Surgeon including the set up of transplant

Interpret and implement current legislation on organ/tissue donation and transplantation

Organise all aspects of cardiothoracic retrieval within the area of responsibility

Participate in a 24 on-call rota
