CAMHS Outreach provides intensive individualised support to children and young people within the community to prevent the need for hospital admission and to respond to urgent situations. We have a fantastic success rate of supporting young people to remain at home and receive the treatment they need. As a result, our small multi-disciplinary team has gained additional investment and we are expanding in size and remit and you will join us at an exciting time of service development.

We are seeking to recruit a Mental Health Navigator to be embedded with CAMHS Outreach to wok with children and young people with moderate to severe mental health needs to support their recovery.

CAMHS Outreach is a multi-disciplinary team, which consists of mental health nurses, children social workers and psychologist and provides intensive short-term support for children and young people (CYP) aged under 18 in their home who are experiencing an acute crisis in their mental health and are presenting with significant risk/crisis behaviours. The service offers an assessment and management of risk with specialist tailored interventions to reduce the escalation in the CYP mental health presentation and prevent first admission or an admission from re-occurring.

The role will include the following;
- Supporting children and young people to access and reintegrate back into either their school, other education provision or local community-based support options and activities.
- Creating good working relationships with a range of practitioners working in Gloucestershire who support mental health and emotional well being needs of children and young people.
- Have an up to date knowledge of support services for children and young people.
- Facilitate supported conversations or targeted, time limited interventions as part of personalised care plans under supervision of CAMHS Outreach Registered Practitioner.
- Support transitions between clinical pathways and agencies

Outreach has now extended its hours to include weekends and bank holidays 9.30 am
