**Responsible To**: Lyndhurst Surgery and New Forest PCN
**Overall Purpose of Job**:
To provide administrative support to Lyndhurst Surgery and the members of the New Forest Primary Care Network (NFPCN) to ensure the effective delivery of care, accurate record keeping, and administrative support to all services relating to patient care.
To ensure patients of Lyndhurst Surgery and across NFPCN receive the co-ordinated care they depend on; care which is responsive to their needs. The postholder will provide reception, administrative, data input, and technical support to Lyndhurst Surgery to ensure the smooth running of their systems and processes and to maximise output supporting patient care.
**Main Responsibilities**:
- Provide proactive operational and administrative support to Lyndhurst Surgery. This will include arranging appointments, signposting to the appropriate service or clinician, liaison with staff, residents, family members and other health and social care professionals, dealing with enquiries and providing and processing information.
- Work as part of the reception team, adhering to the systems, policies and protocol, as set out by the practice.
- Ensure accurate and up to date records are maintained on EMIS Web
- Provide effective appointment monitoring to maximise capacity, utilising appointments according to patient need.
- Support processes for review and obtaining feedback from service users and their families as well as professionals.
- Liaise with all staff across the practice and PCN to promote good communication and ensure the delivery of effective and efficient services.
- Maintain a working knowledge of relevant organisations and agencies to ensure effective joint working, referrals and signposting.
- To ensure up to date information is available on all services including printed and online materials.
- Provide a response to enquiries which is welcoming, person centred and promotes an open and accessible service, strictly in accordance with practice policies, protocols and procedures.
- Demonstrate a sound knowledge and understanding of the needs of people from vulnerable groups and an empathetic approach.
- Other Responsibilities:
- To provide specific support to the Team as and when required and capacity allows.

**The role will primarily be based in Lyndhurst Surgery but may require some travel amongst PCN member practices and business meeting locations**
**Job Type**: Part-time
**Salary**: £12.25 per hour
Expected hours: 18.5 per week
