Patient Pathway Coordinator - Respiratory Department Band 4

An exciting opportunity has arisen for a patient pathway coordinator within the Respiratory Department at Epsom & St. Helier University Hospitals NHS Trust - Cross site.

This is a challenging role responsible for supporting the achievement of the 18-week referral to the treatment target. The role will involve supporting all administrative and clerical issues relating to the patients’ pathway of care tracking and supporting them from referral to treatment/discharge. The role will also involve cross-covering colleagues which will require travel to St Helier Hospital & the Epsom Hospital site.

We are looking for a person with excellent administrative skills, an enthusiastic team player with a flexible approach to working and with a good telephone manner. You should be persuasive, credible with the ability to work in a calm, logical and methodical way whilst being able to multi-task in a busy environment. This is a cross site post.

The post holder will support the provision of the highest quality patient care through dedicated administrative and clerical support and continuous improvement.

Based in a speciality clinical team, the post holder will be the point of contact for all administrative and clerical issues relating to patient's pathway of care, tracking and supporting them from referral to treatment &/or discharge.

The postholder will provide a patient-focused service that supports clinical staff in the provision of efficient integrated pathways that enable the best use of resources and ensure that patients are fit, willing and able to be treated and that they are treated in the right place, by the right person, at the right time.

The post holder will ensure high levels of patient care and will ensure clinician satisfaction by being an accessible, customer focused and knowledgeable point of contact.

Epsom and St Helier University Hospitals NHS Trust provides a range of medical services to approximately 490,000 people living across southwest London and northeast Surrey. In addition, we provide more specialist services, in particular renal and neonatal intensive care, to a wider area, covering parts of Sussex and Hampshire.
