**The Service**:
The Integrated Health and Social Care Learning Disability Team is a part of Adult Social Care - Portsmouth City Council. The team is based at the Kestrel Centre at the Civic Offices. We offer a city-wide health and social care service to adults with learning disabilities and their Carers. The service is delivered in collaboration with the Voluntary and Private Sector and there is active co-production with service users and carers, including the Portsmouth Learning Disability Partnership Board.

**The Role**:
This role involves assessing the needs and strengths of vulnerable adults with Learning Disabilities and their carers, to then identify creative ways to meet their assessed outcomes. The team is committed to pursuing the personalisation agenda, to promote independence, inclusion, choice and rights of service users and their carers. Working in partnership with service users, carers, provider services and other professionals are key elements of the job.

**What you'll need to succeed
**Who is the person?**

You need to:

- Be educated to GCSE A - C in maths and English or equivalent qualification or Literacy/numeracy skill.
- Have experience of working with Adults with Learning Disabilities ideally within a fieldwork setting.
- Be able to advocate for users and carers using a problem-solving approach.
- Have good assessment skills.
- Be motivated to achieve positive outcomes in relation to rights choice, independence and inclusion.
- Be able to acknowledge, respect and respond to individual differences and diversity requirements.
- Have good organisational skills to enable you to manage your workload effectively and ensure your targets and deadlines are met, whilst remaining calm under pressure.
- Communicate sensitively positively and clearly to a wide range of people, in a wide range of formats. This will include the ability to write clear, concise reports and letters and use IT systems.
- Be able to use own initiative, supervision and line management support, and be a wider team player.
