Band 4: Assistant Practitioner, Intensive Support

An exciting opportunity has arisen as part of a recent commitment to increase and develop the Community Learning Disability Intensive Support Team (IST) in South Staffordshire.
MPFT are looking to introduce the following clinical practitioners to an already established, specialist multi disciplinary team:
3 Band 6 Clinical Nurse Practitioners (RNLD) - Full Time Band 6 Occupational Therapist - 4 days / 0.8 WTE1
Band 4 IST Assistant Psychologist - Full Time
1 Band 4 IST Assistant Practitioner - Full Time
The IST forms part of the wider South Staffordshire offer to supporting individuals with a Learning Disability and Autism who are experiencing difficulties which may otherwise lead to placement breakdown or hospital admission. The team is reliant upon effective working relationships between services within and outside of MPFT.
The Intensive Support Team aims to provide a high quality, community based, integrated service for adults with learning disabilities. We are seeking a highly motivated, assistant practitioner to join and help the continuous development of our IST.
We are looking for someone who is enthusiastic, dynamic and committed to multi-disciplinary working. This post will help ensure the delivery of an innovative, community focused specialised service.
Experience of working with people with learning disabilities in the community is desirable, you will also have experience of working with people who have complex health needs, for example those associated with autism and behaviour that challenges, experience of mental and physical ill health is also desirable.

You will be supporting a shared MDT caseload of people requiring flexible, multidisciplinary approaches and participate in the development of evolving, contemporary health care for people who have learning disabilities in Staffordshire with a view to providing care within the person's home and local community. Your role will support IST clinicians and social worker to work in partnership with service users and their carers to support improvements in quality of life, by undertaking person centred assessment and interventions.
You will work in a co-ordinated and planned way, including outside of normal hours if required. The successful applicant will have access to a wide range of training opportunities including Positive Behaviour Support.
The post requires interface with a number of teams and services to promote shared formulation. This includes assessment and management of risks which place the individual and others at risk of serious harm; or for whom the nature or degree of risk might otherwise lead to exclusion, placement breakdown, and admission to inpatient services.
As the post involves travelling across South Staffordshire, having a full UK Driving licence and car will be helpful.
